MDHHS Speaks about MI Health Link in Escanaba – Oct. 20

Lansing, MichiganOctober 19, 2015 – The Michigan Department of Health and Human Services will be hosting a public forum about the MI Health Link program that integrates care for individuals who are dually eligible for Medicare and Medicaid on Tuesday, October 20, 2015, in Escanaba. MDHHS will also provide a conference line for those interested to participate by phone.

Through this forum, MDHHS plans to share information about the status of the program, answer questions, and receive feedback from dually eligible individuals, their families, providers, and other interested stakeholders. These forums will continue on a quarterly basis throughout the demonstration period and will be hosted in the four regions where MI Health Link is available. Developed in partnership with the Centers for Medicare and Medicaid Services, MI Health Link will initially operate for three years.

MI Health Link seeks to improve quality and access to care by more effectively aligning Medicare and Medicaid services and bridging the divide between physical health, long term care, and behavioral health systems. The project takes a person-centered approach in all aspects of the new health care delivery system, and focuses on care coordination, providing access to home and community-based services, ensuring continuity of care during the transition to the new system, and supporting self-determination and choice for individuals.

MI Health Link is operational in four regions of the state: the entire Upper Peninsula; a region in the Southwest part of the state consisting of the following counties: Barry, Berrien, Branch, Calhoun, Cass, Kalamazoo, St. Joseph, and Van Buren; Macomb as a single county region; and Wayne as a single county region. Enrollment is currently being phased-in across the regions. Education, outreach, and enrollment activities will continue throughout the demonstration period.

The Oct. 20 public forum will be held at the Bay College Joseph Heriman University Center located at 2001 N. Lincoln Rd. in Escanaba, Michigan from 10 a.m. to noon.
