Gwinn – Registration for New Elementary Students for The 2014-15 School Year

REGISTRATION FOR NEW ELEMENTARY STUDENTS

FOR THE 2014-15 SCHOOL YEAR

Registration for new students in grades K-5 for will be held on WEDNESDAY, AUGUST 13, 2014 between 8:00 a.m. and 3:00 p.m. All students who have not attended the Gwinn Area Community Schools before should register at KI Sawyer Elementary Schoollocated at 411 Scorpion on the former KI Sawyer Air Force Base. For more information, please call the Sawyer School at 346-5567.

A child entering a Michigan school for the first time must submit an immunization record on the day of registration. Michigan laws also require the immunization record showing the month, day, and year of all shots against diphtheria, tetanus, pertussis, measles, rubella (German measles), mumps, and poliomyelitis. At least four doses of DPT and three doses of polio vaccine are required; and, if a dose was not received on or after the fourth birthday, a booster dose at school entry is necessary. Two MMR (measles, mumps, rubella) received after the first birthday are necessary, with one of the doses at or after fifteen months of age. If the child has not been immunized because of religious convictions or other objections to immunization, a statement to that effect, signed by a parent or guardian, must be presented at the time of registration. A parent or guardian may request that the local health department give the required protective injections and diagnostic tests.

A physical examination is strongly recommended, BUT IS NOT REQUIRED, for each student entering the school district for the first time.

Children entering a Michigan school for the first time must be five (5) years old on or before October 1, 2014. A birth certificate must be presented to verify this age at the time of registration.

REGISTRATION FOR NEW 6-8 GRADE STUDENTS

FOR THE 2014-15 SCHOOL YEAR

Registration for new students in grades 6-8 is available by appointment only. Beginning August 13th, please phone the Gwinn Middle School at 346-5914 to schedule an appointment for registration. Please bring the following to your appointment: last report card, address for last school attended, birth certificate, and immunization records.

All GMS students and their families are encouraged to attend the GMS Orientation/Open House on WEDNESDAY, AUGUST 27, 2014 beginning at 3:00 p.m. in the Gymnasium.

REGISTRATION FOR NEW 9-12 GRADE STUDENTS

FOR THE 2013-14 SCHOOL YEAR

Registration for new students in grades 9-12 is available by appointment only. Beginning August 11th, please call the Gwinn High SchoolGuidanceOffice at346-0310 to schedule an appointment for registration. A copy of the student’s birth certificate and immunization record are required upon registration.

All incoming 9th graders and new students to Gwinn High School are encouraged to attend the New Student Orientation session on WEDNESDAY, AUGUST 27, 2014 at 2:00 p.m. in the Gymnasium. Students should be registered for classes prior to attending the orientation and should plan on attending with their parents/guardians.
