Help others live Healthy Lives with Chronic Conditions: Become a PATH Facilitator! Starting 2/17

PATH Leader Training
Marquette, MI – UPCAP will be offering a 4 day PATH Leader training in Escanaba on February 17th, 18th, 24th, and 25th, 2015.  This training certifies people to conduct the Personal Action Towards Health (PATH) workshops in their communities, along with another trained leader.  PATH is a six week small group program, developed by Stanford University, that teaches practical skills for living a healthy life with a chronic health condition like being overweight, diabetes, high blood pressure, arthritis, fibromyalgia, asthma, chronic pain, or heart disease.  The PATH program focuses on self-care, goal setting, learning ways to deal with not feeling well, and sharing personal experiences with other group members.  Ideal leaders are people living with a chronic health condition themselves or care for someone with a health condition.  Leaders do not need to be health care professionals!  Volunteers will receive assistance with travel expenses.

Contact Tonya LaFave or Tammy Rosa at UPCAP at 1-800-338-7227 for more information or to register.
